Diagnosis, treatment options, and Risk Factors

Once we suspect you are suffering from low testosterone, Dr. Seltzer will order bloodwork to determine your levels, as well as other tests to monitor for side effects. Your level of testosterone and your symptoms will guide the appropriate treatment.

There are a variety of different ways to replace testosterone. These include injections, patches, creams, gels, and capsules. You and Dr. Seltzer will discuss which form is the most appropriate for you.

As with any treatment, there are always risks and side effects. Risks of testosterone replacement therapy include a thickening of the blood, water retention, worsening of sleep apnea, acne, and possibly an increase in prostate size. However, it does not appear that testosterone replacement therapy increases prostate cancer risk.

TRT is a big decision which is why we encourage our patients to read Androgens: Clinical Research and Therapeutics and Testosterone Replacement Therapy and Prostate Cancer.

 Testosterone Replacement Therapy (TRT) FAQ’s

  • Testosterone Replacement Therapy is a medical treatment designed to restore testosterone levels in individuals experiencing symptoms of low testosterone (Low T). It aims to improve energy, mood, muscle mass, libido, and overall quality of life.

  • Common symptoms include low energy, decreased muscle strength, weight gain (especially around the midsection), low libido, difficulty concentrating, mood changes, and reduced exercise performance. A blood test ordered and interpreted by Dr. Seltzer is required for diagnosis.

  • As a physician specializing in hormone optimization and body composition, Dr. Seltzer takes a data-driven, individualized approach—prioritizing symptom relief, safety, and long-term metabolic health rather than just hitting “normal” lab ranges.

  • The process begins with a detailed consultation, medical history review, symptom assessment, and comprehensive lab work. Dr. Seltzer will then discuss results and create a personalized treatment plan based on your goals and physiology.

  • Depending on your needs and lifestyle, therapy may include injectable testosterone, transdermal options, or physician-directed pharmaceuticals that stimulate natural testosterone production. The best method is chosen collaboratively.

  • Most patients begin noticing improvements in energy and mood within a few weeks, with body composition, strength, and libido changes typically occurring over 6–12 weeks. Optimizing levels is a process, not a single adjustment.

  • When managed by a physician experienced in hormone therapy, TRT can be very safe. Regular monitoring of blood levels, hematocrit, PSA, cardiovascular markers, and symptom changes ensures treatment remains both effective and safe.

  • Traditional TRT can suppress natural testosterone production and reduce sperm count. If preserving fertility is a priority, Dr. Seltzer offers alternative protocols to maintain or support fertility while optimizing hormone levels.

  • Initial monitoring is more frequent—typically every 6–8 weeks—as dosing is adjusted. Once stable, follow-ups are generally scheduled every 3–6 months with routine labs to ensure optimal levels and safety.
